9/21/2023 EnchantaVaughn Approved (AM) During morning arrival, client did not display appropriate social behaviors. Client is easily frustrated when he cannot leave class whenever he wants. Client was told to be respectful to his teacher and to treat others how he would want to be treated. Client cheered up when counselor arrived. Client was observed smiling and laughing with peers. Client expressed positive feelings and commented he felt less frustrated and ready to do his work. Client transitioned back into academic environment with positive spirit and cheerful expressions and went to his desk to begin morning assignment. Client displayed good behaviors following one on one. Client was able to switch between each block smoothly without altercation or getting in trouble. (PM) Client went into his lunch period with a nonchalant attitude. Client appeared to be moody and needing one on one. Client was confronted by QMHP. Client was inquired how he was feeling. Client shared negative affirmations at this time. Client have to be willing to make better choices in school in order to achieve all objectives on a consistent basis. Client has shown minimum progress with following treatment plan. Client was seen having difficulty complying to rules and demonstrating inappropriate manners in gym, lunch, and classroom settings. Client behaviors were viewed as aggressive and careless. Client displayed impulsive behaviors due to following behind others who were being disruptive in the hallway. Client displayed defiant and disrespectful manners during transition and needed additional support. The client was assisted in identifying how his body language sometimes contradicts his emotional content. Client did not seem to care and shrugged his shoulders. (EOD) Client displayed a lot of energy before going to see his counselor and begin session. Client was encouraged to settle down and prepare for a group activity. Client displayed some leadership skills and appeared to enjoy being around others. Once client returned to class, he was observed calm and quiet but that quickly changed. Client displayed productive, positivity, polite, and responsible behaviors during evening academic hours. Client appeared to be in a better mood and making better decisions. Although, client displayed minimal particpation going into dismissal. Client lack the skills needed to cope and manage symptoms, and feedback was given in this area. Client was not able to hit his goals listed on his ISP.
